Urinary Tract Infections (UTI) encompass infections anywhere in the urinary tract (including kidneys, bladder, urethra). Most people when referring to UTIs are referring to lower urinary tract infections, which is what this protocol will focus on. Symptoms include urgent / frequent urination, painful or burning urination, and blood present in urine.

What causes it?

UTI's are most commonly caused by the negative gram bacteria E. Coli (between 60-90% of cases), however they can occasionally be caused by other negative gram bacteria, positive gram bacteria, and even candida (1, 2).Due to a shorter urethra, UTI's are far more common in women, and antibiotic, catheter use, diabetes, and sexual activity are all additional risk factors.

Since the majority of infections are caused by E. Coli (with Klebsiella as the second most abundant), and given that these are two of the most common bacterias found with Small Intestinal Bacterial Overgrowth (SIBO), I regard a history of UTI's as a symptom I correlate with SIBO (3) and microbiome imbalances.

Functional Lab Consideration:

Microbial overgrowth/dysbiosis: GI MAP / OAT testing can reveal gut or yeast overgrowth feeding recurrent UTIs.
Blood sugar regulation: Hyperglycemia fuels bacterial growth in the urinary tract.
Nutrient Deficiencies: Address vitamin D, zinc, and selenium status.

Lifestyle Consideration:

Probiotic focus: Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14 (urogenital protective strains).
Hydration: 2–3 liters/day filtered water to dilute urine & help flush bacteria from the urinary tract, reducing bacterial load.
Voiding habits: Urinate before/after sex, don’t hold urine for long periods.

When to seek medical care:

Fever, back/flank pain (possible kidney infection).
Blood in urine, persistent or worsening symptoms after 48 hours of natural care.
Frequent recurrence (>3/year).
