Rheumatoid arthritis develops through complex interactions involving genetic susceptibility, immune function, environmental exposures, nutrition, metabolism, gut health, lifestyle, and individual physiology. This program creates an opportunity to support several of these areas together, including fatty-acid balance, healthy inflammatory signaling, antioxidant defenses, joint health, gastrointestinal and microbiome health, nutrient status, movement, sleep, and stress. How to use this protocol: This program was designed as a complete 12-week nutrition, supplement, and lifestyle framework. If you choose to use only part of the supplement protocol, understand that you are following a modified version of the program rather than the complete protocol described here. The supplements included in this protocol were selected because their ingredients have been studied in areas relevant to rheumatoid arthritis, joint health, immune function, inflammatory balance, antioxidant support, or gastrointestinal health. My functional medicine process is: DETECT → SUPPORT → REASSESS The goal is not to assume that every person with rheumatoid arthritis needs the same supplements or diet indefinitely. The goal is to follow a structured plan, observe how your body responds, and use what you learn to decide what should come next. Step 1: Choose ONE Nutrition Strategy You do not need to follow all three diets. Choose one approach that fits your goals, health status, and ability to follow it consistently. Option 1: Elimination Diet This is the most structured option and is especially useful when you want to investigate whether particular foods may be contributing to how you feel. The attached Elimination Diet Guide explains how to remove common potentially reactive foods and then reintroduce foods systematically. The accompanying 14-Day Elimination Diet Meal Plan gives you two complete weeks of meals, grocery lists, recipes, and planned leftovers to make implementation easier. The goal is not permanent food restriction. The goal is to create a period of consistency, observe your response, and then reintroduce foods methodically so you can begin building a more individualized long-term diet. Option 2: Mediterranean Diet This is the least restrictive of the three options and may be the easiest to maintain long term. The Mediterranean approach emphasizes whole and minimally processed foods, vegetables, fruit, olive oil, fish, legumes, whole grains, nuts, and seeds. It has also been studied in rheumatoid arthritis and cardiovascular and metabolic health. Because it includes foods such as grains, dairy, legumes, eggs, and nightshades, it does not create the same food-elimination experiment as the elimination diet. It is a good option when sustainability and dietary quality are the primary goals. Use the attached Mediterranean Diet Guide as your implementation resource. Option 3: Ketogenic Diet The ketogenic diet is a more structured metabolic approach that substantially reduces carbohydrate intake and increases fat intake so that the body begins producing ketones and using fat as a major fuel source. The attached Fullscript ketogenic guide generally targets approximately 20 to 50 grams of carbohydrate per day , and the accompanying one-week meal plan provides meals, grocery lists, recipes, and planned leftovers. This approach may appeal to someone interested in carbohydrate restriction, metabolic health, appetite regulation, or the physiological effects of nutritional ketosis. The ketogenic diet is not the same as an autoimmune elimination diet . It can include foods such as dairy, eggs, nuts, tomatoes, peppers, and other foods that may be removed during an elimination strategy. People taking glucose-lowering medications, or those with certain liver, pancreatic, kidney, or metabolic conditions, should discuss a ketogenic diet with an appropriate healthcare professional before beginning. Fullscript's guide also discusses hydration, electrolytes, and other considerations during the transition into ketosis. Step 2: Make the Plan Practical Two additional resources are included to help you turn information into action. Goal-Setting Strategies & Worksheet Use this first to identify one or two realistic goals for the program. The worksheet uses the SMART framework: Specific, Measurable, Attainable, Realistic/Relevant, and Time-bound Do not try to change everything at once. Choose one or two areas that matter most to you, such as nutrition, movement, sleep, supplement consistency, or stress management. My Action Plan After choosing your goals, use the Action Plan to define exactly what you are going to do, when and where you will do it, and how you will follow through. The worksheet specifically includes nutrition, supplements/medications, physical activity, sleep, stress reduction, and other health behaviors. Step 3: Follow the 12-Week Supplement Protocol Follow the supplement directions listed with each product. Pay attention to tolerance and how you respond over time. Do not assume that every supplement should automatically be continued indefinitely. The supplement protocol is designed around a 12-week foundational phase , but individual supplements may have shorter recommended durations. Step 4: Reassess At approximately 12 weeks, review what has changed in areas such as: joint comfort and stiffness mobility and physical function energy digestion food tolerance sleep activity overall well-being If laboratory testing or other health information is available, those findings may also help determine whether supplements should be stopped, continued for another defined period, adjusted, or replaced with more targeted support. Do not automatically continue the entire protocol beyond 12 weeks without reassessment. A diagnosis tells us what disease has been identified, but it does not automatically tell us what triggered the autoimmune process, what may still be driving it, or which factors may be changed. I created this 12-week nutrition and supplement foundations program to support the physiology that can matter in Hashimoto’s while you make changes to your diet and lifestyle. The goal is not to rely on one nutrient or one supplement. Hashimoto’s is more complex than that. My approach follows a simple process: DETECT → SUPPORT → REASSESS Detect: Pay attention to the factors that may be influencing your autoimmune and thyroid health, including diet, digestion, gut and barrier health, nutrient status, blood-sugar patterns, sleep, stress, activity, environmental exposures, medications, and your existing laboratory information. Support: Use a focused nutrition, supplement, and lifestyle strategy to support thyroid physiology, antioxidant defenses, digestion, the intestinal barrier, fatty-acid balance, vitamin D status, sleep, recovery, and overall immune regulation. Reassess: At the end of approximately 12 weeks, look at what changed, what did not, and what your response teaches you. Supplements should not simply continue forever without asking whether they are still needed. Nutrition Comes First Supplements work best when they are part of a broader strategy. For most people with Hashimoto’s, I prefer starting with a structured elimination-style diet because it gives you an opportunity to remove common problem foods for a period of time and learn from how your body responds. An autoimmune paleo-style approach can also be useful because it removes foods such as gluten, dairy, grains, legumes, and nightshades during the elimination phase. A ketogenic diet can be another option, particularly when blood-sugar regulation, insulin resistance, appetite, or metabolic health are important concerns. A Mediterranean-style diet can also be a very healthy approach. If you choose it, I encourage you to pay attention to your individual response to foods that may still be included in a traditional Mediterranean diet, such as gluten-containing grains, dairy, legumes, or nightshades. I have included several dietary guides and meal plans with this program so you can choose a strategy you are realistically able to follow. You do not need to follow every diet at once. Pick one approach, be consistent, and learn from your response. If you have celiac disease , gluten avoidance is medically necessary. If you do not have celiac disease, a gluten-free elimination period can still be useful as part of a structured dietary trial. Why These Supplements Are Included This protocol was designed to support several areas that may be important in Hashimoto’s rather than simply supplying a long list of vitamins. The thyroid-focused formulas provide nutrients and compounds involved in thyroid physiology, selenium status, myo-inositol pathways, antioxidant defense, glutathione support, and inflammatory balance . Omega-3 fatty acids provide EPA and DHA to support healthy inflammatory balance and fatty-acid status. Vitamin D is included because vitamin D status can be important in immune and thyroid health. Whenever possible, I prefer vitamin D supplementation to be guided by testing rather than assuming everyone needs the same dose. Glutathione support is included to help support antioxidant defenses. Digestive enzymes are taken with meals to support more complete digestion of food proteins. The included enzyme formula contains DPP-IV activity, but digestive enzymes do not make gluten safe for someone with celiac disease and should never be used as permission to intentionally consume a food you know you need to avoid. Mucosal-barrier support is included for the gastrointestinal lining because gut and barrier health can be an important part of the autoimmune picture. A modest amount of magnesium in the evening is included to support magnesium status, relaxation, sleep, and recovery. A Special Note About Iodine I do not routinely recommend supplemental iodine for Hashimoto’s . More iodine is not automatically better for the thyroid, and excessive iodine exposure can increase oxidative stress within the thyroid and may aggravate autoimmune thyroid activity in susceptible people. While following this program, avoid adding iodine, kelp, seaweed concentrates, or iodine-containing “thyroid support” supplements unless there is a specific reason to use them and you are being appropriately monitored. This does not mean everyone needs an extremely low-iodine diet. The goal is to avoid unnecessary supplemental iodine loading while keeping the overall approach individualized. Do Not Add Extra Selenium The thyroid formulas included in this program together provide approximately 200 mcg of selenium per day . Do not add additional high-dose selenium supplements, selenium-containing thyroid formulas, or large amounts of selenium-rich foods specifically for the purpose of increasing selenium intake unless a qualified healthcare professional has reviewed your total intake. More is not necessarily better. Thyroid Medication If you take levothyroxine, desiccated thyroid, T3, T4, or another thyroid medication, continue taking it exactly as prescribed. Do not reduce, stop, increase, or otherwise change thyroid medication because you started this program or because you begin feeling different. Thyroid medication timing can also matter because food, minerals, supplements, and medications may affect absorption. Follow the instructions given by your prescribing clinician or pharmacist and ask them how far apart your thyroid medication and supplements should be taken. Lifestyle Still Matters Hashimoto’s does not occur in isolation from the rest of your physiology. Sleep, stress, physical activity, blood-sugar regulation, digestive health, nutrient status, and environmental exposures can all influence how you feel and how well your body functions. I have included lifestyle recommendations for: nutrition, movement, sleep, stress management, goal setting, and creating an action plan. You do not need to change everything perfectly on day one. Consistency with a smaller number of meaningful changes is usually more useful than trying to do everything at once and becoming overwhelmed. Your 12-Week Reassessment This protocol is designed as a 12-week foundational phase , not an indefinite supplement program. At approximately 12 weeks, reassess: How are your energy, digestion, sleep, mental clarity, bowel function, exercise tolerance, and overall well-being? Which dietary changes appeared helpful? Which supplements seem worth continuing? Which may no longer be necessary? Would repeat vitamin D, thyroid antibodies, thyroid-function testing, fatty-acid testing, gastrointestinal testing, or other evaluation help answer the next question? What did your response teach you? Some nutritional strategies may make sense to continue. Others may be reduced, discontinued, or replaced with something more individualized.
