    This is a long list of products, you may not need all of them. Foundation first: Fiber + magnesium + D/K2 + omega-3s = a powerful baseline. Symptom-specific add-ons: Hot flashes: Black cohosh, berberine (if metabolic issues also exist). PMS/early perimenopause: Vitex. Stress/fatigue: Ashwagandha, rhodiola. Metabolic syndrome: Berberine, gymnema. Caution with wild yam —it's not a substitute for Rx progesterone which is well controlled and standardized (it's also bioidentical) The ranked list: 1. Fiber (Soluble & Insoluble) Why it's #1 : Fiber isn’t specific to menopause but is critical for overall health—gut microbiome support, blood sugar balance, and weight management (which become trickier post-40), plus estrogen metabolism through binding excess estrogen in the gut. Key symptoms addressed : Weight gain, insulin resistance, constipation, bloating, estrogen dominance. Evidence : Women with higher fiber intake have lower circulating estrogen levels ( Gaskins et al., 2009 ). Fiber intake is associated with improved insulin sensitivity and reduced visceral fat ( Weickert & Pfeiffer, 2018 ). 2. Magnesium Glycinate Why it's high on the list : Magnesium is involved in over 300 enzymatic reactions, and glycinate is particularly good for calming, sleep, and muscle relaxation—huge for perimenopause when insomnia, anxiety, and restless muscles are common. Key symptoms addressed : Sleep disturbances, anxiety, muscle cramps, migraines, mood swings. Evidence : Magnesium helps alleviate insomnia and mood disturbances in menopausal women ( Cuciureanu & Vink, 2011 ). Some evidence for reducing hot flashes, especially when combined with other interventions ( Khine et al., 2006 ). 3. Vitamin D3 + K2 Crucial foundational: Vitamin D is key for bone health, mood, and immune function, while K2 helps direct calcium to bones and away from arteries. This combo is essential, especially as estrogen drops. Key symptoms addressed: Osteopenia/osteoporosis, low mood, immune dysregulation. Evidence: Strong evidence for fracture reduction and bone health (Avenell et al., 2014), and mood regulation (Penckofer et al., 2010). Avenell A, et al. (2014). Vitamin D and K2 for bone health. Omega-3 Fatty Acids (EPA/DHA) Well-established support: Omega-3s have anti-inflammatory benefits, improve cardiovascular health, and may slightly reduce hot flashes. They're essential for brain and mood health, particularly relevant during menopause. Key symptoms addressed: Hot flashes, mood swings, joint pain, cardiovascular risk, brain fog. Evidence: Meta-analyses show modest improvement in hot flashes and strong cardiovascular protection (Lucas et al., 2009; Tartibian et al., 2010). Lucas M, et al. (2009). Omega-3 and menopausal symptoms: A pilot study. Menopause . Tartibian B, et al. (2010). Omega-3 supplementation and menopausal symptoms: Menopause . 5. Chaste Tree (Vitex agnus-castus) Why it's key : Best evidence for early perimenopause where PMS-type symptoms (cyclical breast tenderness, irritability, irregular cycles) still occur due to luteal phase defects. Works via dopaminergic pathways to reduce prolactin and potentially balance progesterone. Key symptoms addressed : Breast tenderness, mood swings, irregular cycles, PMS. Evidence : Meta-analysis shows efficacy for PMS and cyclical mastalgia ( Shahnazi et al., 2017 ). Evidence is weaker in postmenopause 6. Black Cohosh Classic herbal support: Black cohosh is one of the most studied herbs for menopause, acting primarily via serotonergic pathways (not hormonal). Best suited for vasomotor symptoms like hot flashes. Key symptoms addressed: Hot flashes, night sweats. Evidence: Multiple RCTs support efficacy for vasomotor symptoms (Beer & Neff, 2013), though results are sometimes mixed. Beer AM & Neff A. (2013). Black cohosh review. Adv Ther . Nappi RE, et al. (2005). Black cohosh vs. placebo: Maturitas. 7. Berberine Why it matters : Stellar for metabolic syndrome—a big concern in midlife due to falling estrogen and rising insulin resistance. Functions similarly to metformin: AMPK activation, improved insulin sensitivity. Key symptoms addressed : Weight gain, blood sugar instability, lipid abnormalities. Evidence : A meta-analysis confirms berberine improves glucose metabolism and lipid profiles ( Lan et al., 2015 ). May indirectly reduce androgenic symptoms (like facial hair) by improving insulin resistance. 8. Ashwagandha (Withania somnifera) Why it's here : Excellent adaptogen for stress support; moderate-quality evidence for reducing anxiety, sleep issues, and perceived stress, which spike in menopause due to HPA axis changes. Key symptoms addressed : Anxiety, fatigue, sleep disturbances, cognitive fog. Evidence : RCTs show significant reductions in stress and cortisol ( Chandrasekhar et al., 2012 ). Some support for sexual function and mild improvement in vasomotor symptoms ( Lopresti et al., 2019 ). 9. Rhodiola Rosea Adaptogen booster: Rhodiola helps the body adapt to stress and may enhance mental clarity and physical endurance—valuable for managing menopausal fatigue and fog. Key symptoms addressed: Fatigue, low mood, brain fog, mild anxiety. Evidence: RCTs show significant fatigue reduction and improved cognitive performance (Darbinyan et al., 2000; Spasov et al., 2000). Darbinyan V, et al. (2000). Rhodiola and fatigue: Phytomedicine . Spasov AA, et al. (2000). Mental performance under stress: Phytomedicine. 10. Gymnema Sylvestre Why it’s useful : Another metabolic helper, excellent for blood sugar balance and sugar cravings, though evidence is less robust than berberine and more focused on diabetics. Key symptoms addressed : Blood sugar spikes, sugar cravings. Evidence : Can reduce sugar absorption and lower blood sugar ( Leach, 2007 ), but not a stand-alone solution. 11. Progesterone (Wild Yam Cream) Caveat-heavy territory : Wild yam cream contains diosgenin, which does not convert into progesterone in the body despite marketing claims. Prescription bioidentical progesterone (micronized) DOES work and is a different conversation. Key symptoms addressed : If bioidentical: sleep, anxiety, uterine protection (with estrogen). If wild yam: Likely placebo effect. Evidence : Wild yam topical use hasn’t shown measurable hormonal effects ( Komesaroff et al., 2001 ). Micronized progesterone (Rx) is effective for hot flashes, sleep, and anxiety ( Hitchcock & Prior, 2014 ).

However, you can find most of these items at a local pharmacy or grocery store (Target, CVS, RiteAid, Walgreens, etc.). *For those with Celiac disease, gluten sensitivity, sensitivities to dyes/fillers, check ingredients before buying. You may be better off with the brand name or a different generic formulation than the options below. *Please remember to consult with your Doctor prior to adding any supplements or herbs to your regimen, this ensures safety and prevents negative interactions with any medications* PLEASE READ THIS SECTION FIRST 1. This is a general guide to items that may be helpful if you catch Covid19 -- this list is based on literature, my experience, and the different theories of Long COVID derived from existing research (i.e. microclots & platelet activation, autoimmunity, viral persistence, etc.) which I discuss and link to in the article in #2, so you can have a better sense of why particular things are recommended, and which have stronger vs. weaker evidence for their use: 2. Where items are not available on Fullscript, I've tried to put links to the most affordable options I could find from reputable vendors. 6. You can search for other options besides those listed below within the Fullscript catalogue. I tried to pick the options that had the least problematic fillers and were still well priced. If you don't have difficulties with magnesium stearate or silicon dioxide, there are likely cheaper options for each item within the Fullscript catalogue and at local retailers. 7. Feel free to shop around for better prices from reputable vendors elsewhere, just be careful purchasing from off-brand stores on Amazon as fake/non-authentic items and poor temperature control during warehouse storage can impact the quality and effectiveness of the items you buy. If you can, try to buy cGMP certified supplements made in the USA. 8. This goes without saying, but do not take anything you are allergic or sensitive to. Do not take aspirin, nattokinase, serrapeptase, or lumbrokinase if you are on prescription blood thinners or have a bleeding disorder, stomach or intestinal ulcers, recent surgery, are pregnant or breastfeeding. Cucurmin can interfere with Cytoxan and 2 other chemotherapy drugs. Alpha lipoic acid can interact with diabetes drugs (not metformin) and interfere with thyroid conditions. 9. It is SO much harder to get care, into trials, etc. without the proof of positive test! PLEASE NOTE: Some people take a long time to test positive, or they never do, so act/treat yourself as if you have Covid19 if you had a known or suspected exposure and you have symptoms, regardless of whether your test is positive. LIMITING SPREAD OF COVID TO OTHERS IN YOUR HOME IF YOU CATCH IT: - distancing from others in your household if you can, consider using a separate bathroom or sleeping in a different bedroom as others until you are recovered - Run HEPA filters in your room with the room door closed, - wear a well fitted N95 or better mask in common/shared areas - leave windows open to the outside for better air flow (weather and allergies permitting). - allow natural sunlight in (UV helps kill Covid19 on surfaces) - wash hands often, avoid touching or kissing others' faces until you are recovered - avoid sharing utensils, cups, or food with others until you are recovered REST, SLEEP & PACE YOURSELF #. REST, SLEEP: If you need time off work to recover (highly recommended), ask your provider for a note. If your body is telling you to sleep, do so (when safe to do so, of course). If you are struggling to sleep, consider sublingual melatonin (see below). #. PACE your activities - do not overdo it. Even if you feel better quickly, ease back into strenuous activities gradually. Re-starting intense activities or exercise too soon after Covid19 infection kicked-off Long COVID symptoms in many people. After you feel fully recovered, give yourself another week at least before you can do some gentle exercise (a short walk perhaps), and gradually increase intensity and duration of exercise as tolerated over the next 2-3 weeks** back to your prior baseline only as tolerated. **If you find that your resting heart rate is notably higher than it was prior to Covid19 infection, or if you heart rate is shooting up high with minimal activity, like standing up, walking across your room, or up a flight of stairs, you will need to have an even more gradual progression back to pre-illness activity levels (months, not weeks)! And it is possible that taking 600mg of ALA (alpha lipoic acid) right away when you notice the increased heart rate could help reverse the issue (but it has to be taken right away, it's a very short window of time, so good to have this on hand ahead of time). a a
