Subject: Supporting Your Weight Loss Journey: Transitioning from GLP-1 Medications AVAILABLE TO DISCUSS WITH YOUR PRIMARY CARE PROVIDER Not intended to use without support from a provider. Hi there From PA Nicole, As you begin to transition off prescription GLP-1 medications, I want to ensure you feel supported with the right tools to maintain your weight loss and continue moving toward your health goals. GLP-1 medications have been a powerful jumpstart, but long-term success comes from building sustainable habits supported by high-quality supplementation, a low-carb nutrient-dense diet, and an active lifestyle. Below is a recommended supplement plan designed to optimize your metabolism, support appetite control, reduce inflammation, balance glucose, and help maintain your progress. Plan to start these before stopping the GLP-1 shots (in the last month if possible): Days 1-5: 1. ProThrivers™ Wellness Multivitamin A well-rounded, oncology-formulated multivitamin that supports daily cellular health, stress resilience, and nutritional repletion—important as we shift away from pharmaceutical support. 2. Berberine + Chromium A natural alternative to metformin that improves insulin sensitivity and supports healthy lipid metabolism. One below : OR https://equi.life/products/berberine?irad=1923800&irmp=7061522 3. MetaFiber® A blend of soluble and insoluble fiber that helps with satiety, stabilizing blood sugar, and supporting regular bowel movements—especially important during dietary transitions. Then ….. Days 5-10 add to above daily: 4. Calocurb Amarasate Appetite Control Plant-based support for appetite regulation via gut-brain signaling. It mimics some of the satiety effects of GLP-1 naturally. 5. Strength by InfiniWell Adaptogenic formula to support energy, stress response, and hormone balance—especially helpful as the body adjusts to life post-GLP-1. 6. GLP-1 Probiotic Pro (Pendulum) Contains Akkermansia and other strains shown to enhance the body’s natural GLP-1 production and maintain metabolic health post-medication. Then… days 10-15 add to above daily: 7. Creatine (Thorne) Supports lean muscle maintenance, strength, and metabolic function—especially important when paired with resistance exercise. 8. L-Glutamine (Lidtke) An amino acid that supports gut lining repair, reduces sugar cravings, and helps preserve lean muscle mass. 9. Glucose Control Pro (Pendulum) Supports healthy glucose levels with targeted probiotics and polyphenols—ideal for maintaining glycemic control after stopping GLP-1s. Then… days 15-20 add to above daily: 10. MegaMarine (Gut-Specific Fish Oil) High in EPA/DHA and designed to support gut health and reduce systemic inflammation, which can improve insulin sensitivity and support fat metabolism. 11. Weight Management Formula (Professional Formulas) A gentle homeopathic option to support metabolism, fat burning, and appetite regulation. For improving weight loss efforts by temporarily relieving increased appetite or cravings, occasional headache, and irritability due to a restricted diet. Consider adding these as protein and electrolyte options: 12. Plant Protein+ or ProVeg Organic Pea Protein Clean, plant-based proteins to help maintain muscle mass and keep you full—ideal for meal replacement or post-workout recovery. Offering 15g of protein, 4g of carbs and 0g of sugar, Plant Protein+ is an easy and delicious way to power your day. It’s great post-exercise to support muscle strength and recovery or to fortify your daily diet. AND KETO Electrolyte Powder (Trace Minerals) Supports hydration, energy, and electrolyte balance, especially helpful during carb restriction or intermittent fasting.Keto friendly - 0g Carbs - Replenishes lost electrolytes† - Rapid rehydration† - Sustained energy & endurance† - NO muscle cramps† - Efficient muscle recovery† - Full spectrum trace mineral & electrolyte complex - Delicious flavor OR Any other favorite protein powder OR electrolyte drink. How to Begin: • The Month before stopping shots: Start with 2-3 new supplements every 3–5 days to allow your body to adjust. This helps reduce the chance of digestive discomfort and ensures optimal absorption. • Take all products with food unless otherwise instructed. Morning or lunchtime is often ideal to support energy and appetite regulation. • Intermittent Fasting: If you’re not already practicing it, begin intermittent fasting at least 2 weeks before stopping GLP-1. A common approach is the 16:8 method—eating all meals within an 8-hour window and fasting for 16 hours (e.g., 11 AM–7 PM). This helps your body maintain insulin sensitivity and fat-burning rhythms. • Low-Carb Diet: Transition to a low-carb, whole-food-based diet at least 2 weeks before stopping the medication. Focus on protein (chicken, turkey, eggs, fish), healthy fats (avocado, olive oil, nuts), and fiber-rich vegetables (leafy greens, cruciferous veggies). Avoid processed carbs and sugars to stabilize blood glucose and hunger signals. Aim for 90g protein a day. • Hydration: Aim for at least 80–100 oz of water daily. Adding electrolytes (like in the Keto Electrolyte Powder) can prevent fatigue, headaches, and muscle cramps—especially during carb restriction or fasting. • Movement: Stay physically active with a goal of 30 minutes per day, at least 5 days a week. This can include brisk walking, weight training, yoga, swimming, or cycling for example—anything that helps maintain muscle and supports mood, metabolism, and glucose control. • Continue these supplements for 5-6 months to give your body the best chance to recalibrate after GLP-1 therapy. Note: These habits need to become routine , and will help your body transition off the GLP-1 medication more smoothly and ensure the supplements work effectively to support your ongoing weight and health goals. Remember, weight loss is about more than medication. It’s about creating a new foundation for long-term health, and this plan is built to do just that—with the right fuel, nutrients, and lifestyle support. I understand that this supplement plan may seem like a $$$$💸 financial investment , but it’s important to recognize that the long-term cost of GLP-1 injections—often ranging from several hundred to over a thousand dollars per month—far exceeds the cost of this targeted, nutrient-rich support. These supplements are carefully chosen to work synergistically to help your body function optimally, reduce cravings naturally, and maintain weight loss without ongoing pharmaceutical dependency. This approach not only supports your health more holistically but is also far more sustainable financially over time. ps. please also see the attachments for support during this transition. Kindly, In health, Dr. Nicole Hoff, DMS, PA-C

Dear patients, parents, and sick friends and family! This list is going to seem overwhelming AND expensive at first, but this is a very calculated protocol of what to ”have on hand“ and in your “wellness/ first aid kit” tool box during times of viral and/or bacterial illness. I assure you this kit will treat your entire family in an acute situation—see it as cheaper than several urgent care or ER visits! You (or your child) may NOT be taking these ALL at once, but they are very helpful when illness hits the home for you to have on hand because I WILL MENTION these and I prefer you have them on hand when a bug hits in the middle of night and you do not feel well enough to get to the store or cant get fast shipping! My Tele-health fee for a viral illness is $75 for existing patient, $150 for new patient. Based on: I-CARE Early COVID Treatment Summary · March 30, 2023 Infection with SARS-CoV-2, the virus that causes COVID-19, progresses through a number of stages and phases. Treatment is therefore highly stage-specific and should be initiated immediately after the onset of flu- like symptoms. A confirmed antigen or PCR test is not required. The multiple therapies and drugs in this protocol have different mechanisms of action and work synergistically during various phases of the dis ADULT First Line Therapies: (In Order of Priority; not ALL required) https://covid19criticalcare.com/wp-content/uploads/2023/02/I-CARE-Early-Covid-Treatment-Summary-2023-10-16.pdf ——————————- Prescriptions ( contact Nicole PA, for a tele-med visit and prescription ) 1. Ivermectin: 0.4 to 0.6 mg/kg – one dose daily for at least 5 days or until symptoms resolve. If symptoms persist longer than 7 days, consult a healthcare provider. See Table 1 for help with calculating correct dose. Due to a possible interaction between quercetin and ivermectin, these drugs should be staggered throughout the day (see Table 2). For COVID treatment, ivermectin is best taken with a meal or just following a meal, for greater absorption. 2. Hydroxychloroquine (HCQ): 200 mg twice a day for 5 to 10 days. Best taken with zinc. HCQ may be taken in place of, or together with, ivermectin. While ivermectin should be avoided in pregnancy, the FDA considers HCQ safe in pregnancy. Given the pathway used by the Omicron variant to gain cell entry, HCQ may be the preferred drug for this variant. 3. Z-Pack: If needed for a secondary infection that is bacterial. 4. Steroids/saline/nebulizer: F or Inhalation if needed for continuous cough. ———————————— Over-The-Counter, Online/HEB/CVS/Etc or ORDER HERE! Feel free to price shop. 1. Mouthwash : 3 times a day. If premixed product is not available, dilute the more widely available 10% solution (see box) and apply 4-5 drops to each nostril every 4 hours. **Pour 1 1⁄2 tablespoons (25 ml) of 10% povidone-iodine solution into a 250 ml nasal irrigation bottle; fill bottle to top with distilled, sterile, or previously boiled water.
